This Multidimensional Application is based on algorithms from University of Michigan CVS and consists of 3 main parts: Warfarin Dosing Calculator, Warfarin Dosing Information, and a Patient Database.
The Warfarin Dosing Calculator includes the following:
# Calculates Dosages for both Initial and continued Dosing
# The Initial Dosing Calculator utilizes 2 drop down lists representing the Day of Therapy and the Patient's Current INR range and quickly calculates the Recommended Dose Range
# Continued Dosing calculator uses 2 drop down lists representing Target and Current INR and the current weekly or daily dose and calculates both a new daily and weekly dose
# Calculator lists information related to the calculation
# A "loop" takes the calculated dosages and gives the actual pill dosages based on the current pill strengths available on the market today.
The Warfarin Dosing Information consists of 9 separate forms and include the following:
# Initial Dosing Information
# Follow Up Algorithms
# Dosing Guidelines
# INR ranges based on Condition being Treated
# treatment of Patients with INR 5 - 9
# Treatment of Patients with INR > 9
# High INR Values with Minor Bleeding
# High INR Values with Active Bleeding
# Meds that cause an Increase in PT/INR
# Meds that cause an Decrease in PT/INR
Patient Database includes the following:
# Patients are stored individually by Name and DOB
# Database stores individual Patient's INR, PT, Current Dose and calculates and stores Recommended Dose
# Once Initial Dosing Information entered Application calculates and Stores future Recommended Doses based on Target and Current INR's
# User is left with just entering Lab Date and INR (PT Optional)
